Check the specific plan's network
An HMO usually focuses care within its network and may require referrals; a PPO usually allows out-of-network care at a higher cost. An EPO generally limits care to its network with exceptions, such as emergencies. Confirm the contract rules and the provider's participation for the exact policy.
